As a Service Coordinator (Mental Health Case Manager), you will play a pivotal role in our Vallejo campus, which is based on a commitment to providing mental health clients with a continuum of care that puts them well on the road to recovery.
In this position, you will assess clients to identify individual problem areas necessitating treatment and complete comprehensive assessments. You will coordinate interdisciplinary team meetings and discharge planning efforts among the multidisciplinary treatment team. Working closely with nursing staff, you will coordinate the overall treatment plan implementation for clients on the unit to ensure continuity of treatment efforts.
