The Service Coordinator is a vital member of the Birth to Three Program, dedicated to supporting children and families by managing an assigned caseload and ensuring timely access to essential services. This role goes beyond administration; it enhances family quality of life by connecting families to resources and fostering their engagement in programs that promote their child's physical, social, and developmental well-being.
As a core member of the Primary Coach Approach to Teaming (PCATT) model, the Service Coordinator collaborates closely with multidisciplinary team members, including therapists, social workers, and teachers, to deliver individualized, family-centered services. Working under the Assistant Director of Birth to Three Services, you will partner with parents and guardians to support the family in accomplishing the goals identified on the child's Individual Family Service Plan (IFSP).
Your work is defined by a balance of direct family support and strategic coordination. You will provide intensive family support services, utilizing coaching strategies to address frustrations, concerns, and needs. A significant portion of your time involves coordinating the assessment process, facilitating IFSP development and review meetings, and scheduling necessary home visits.
You will serve as the primary point of contact, maintaining monthly contact with all families on your caseload to facilitate communication and ensure the timely provision of services. This includes assisting families in identifying service providers, locating community resources, and facilitating transitions to the Local Education Agency (LEA). You will also act as an advocate for families at Individual Education Plan (IEP) meetings when needed, while ensuring all documentation and billing are accurate and compliant with state and agency standards.
