About the Role

The Service Coordination Manager (SCM) is the vital link between our branch office, our patients and families, and our caregivers. This role exists to ensure that the right nurse skillset matches the clinical needs of the patient while building strategic relationships with Payroll, Finance, Human Resources, leadership, and our clients.\n In this position, you will facilitate seamless communication and conduct home visits to understand the unique environment and personalities of our patients. This personal insight allows you to match families with caregivers as closely as possible. You will manage the full lifecycle of staffing, from interviewing candidates and supporting the onboarding process to maintaining accurate records of time worked and securing insurance authorizations. Your work ensures that our field staff are supported and that our patients receive consistent, high-quality care.

Key Responsibilities

  • check_circleConduct weekly client calls to inform families about schedule updates and changes
  • check_circleImport authorizations from RCM into the EMR system and submit authorization request forms
  • check_circleManage key performance indicators to enhance operational efficiency and drive business growth
  • check_circleMaintain a current patient roster and monitor all schedule changes
  • check_circleBuild and fill shift schedules matching nurse skillsets to patient clinical needs
  • check_circleConduct home visits to assess patient environments and match families with caregivers
  • check_circlePerform telephone screens and face-to-face interviews with potential field clinician candidates
  • check_circleReview nurse and patient records to ensure documentation meets QAPI audit guidelines
  • check_circleVerify compliance of completed shifts and match notes to charges for accurate payroll

Requirements

  • verifiedTB screening with 2 step PPD, or 1 step PPD with proof of 2 step PPD administration within the last year (Pennsylvania)

Nice to Have

Associates degree. Healthcare experience, particularly in home care or related fields. Experience organizing and managing office files, logbooks, and staff schedules. Ability to establish and maintain effective channels of communication with patients and field clinician team members. Ability to receive and execute a high volume of phone calls utilizing appropriate phone etiquette. Ability to manage multiple projects and initiatives simultaneously, ensuring timely completion and quality outcomes.
