Collectively, the Care Synergy network generates approximately $100 million in annual revenue and serves thousands of individuals and families each year through hospice, home health, palliative care, CAPABLE, wellness, and PACE programs. Affiliated programs include The Denver Hospice, Pathways Hospice, Pikes Peak Hospice and Palliative Care, Colorado Visiting Nurse Association (CVNA), CAPABLE, and Superior Medical Equipment, along with PACE programs including Colorado PACE and Pathways PACE.
Colorado Visiting Nurse Association (CVNA) has an immediate opening for a Wellness RN/Medical Foot Nurse. This role is responsible for performing direct patient care at all types of Community Wellness Clinics, including Medical Foot Care, Immunizations, and Corporate Health sites. The position is based on a wellness model using the principles of health promotion, health monitoring, health maintenance, and disease prevention.

Mileage and expense reimbursement is provided at sixty-two and a half cents per mile, one of the highest rates in the industry.
