This position provides oversight of all patient care services and personnel within the interdisciplinary team. The role is designed to ensure the highest quality of care by managing patient and personnel assignments, coordinating referrals, and facilitating continuous assessment of patient needs. A key focus is the development, implementation, and ongoing updates of individualized plans of care.
Day-to-day, you will coordinate appropriate patient assignments and review required documentation. You will serve as a vital link, facilitating interaction between field staff, office staff, home health patients, and their providers. This role exists to ensure seamless care delivery and to support the team in thriving within our mission-driven environment.
