About the Role
We are seeking a Full-time Weekend Registered Nurse to join our field team. As a Home Health Weekend RN, you will serve as the primary point of contact for all disciplines involved in providing care to patients. Your day will involve performing skilled nursing visits, coordinating client care, and overseeing the frequency of visits for each episode. You will consult with physicians regarding patient care details, ensure accurate OASIS collection and medication profiles, and facilitate timely reporting of lab values. Additionally, you will actively participate in weekly case conferences and monthly case manager meetings to ensure comprehensive patient support.
