About the Role

As a Registered Nurse (RN) in Home Health, you will serve as the central point of contact for all disciplines involved in providing care to patients. Your role involves overseeing the frequency of visits for each episode and coordinating client care. You will consult with physicians and the office as needed to provide detailed updates on patient care. Key aspects of your day-to-day include performing accurate OASIS collections, ensuring medication profiles remain current, and timely reporting of lab values to physicians. Additionally, you will actively participate in weekly case conferences and monthly case manager meetings to ensure comprehensive care delivery.
