The Emergency Department Case Manager serves as a vital bridge within the healthcare continuum, dedicated to optimizing patient throughput and ensuring the most appropriate level of care is delivered efficiently. This role is designed to facilitate interdisciplinary collaboration across the Emergency Department, inpatient, and outpatient settings, working closely with patients, families, physicians, community resources, and payers.
In this position, you will leverage advanced knowledge of patient status determination (such as MCG Criteria) and clinical care processes to assess and manage patient needs. Your scope includes identifying the right care setting—whether Inpatient Admission, Observation, or Outpatient status—while leading discharge planning and resource management. Beyond direct patient coordination, you will actively participate in developing clinical practice guidelines and physician order sets, utilizing your expertise in pathophysiology and pharmacology to improve quality of care and reduce costs.
This is a PRN (as needed) position that includes weekends and occasional holidays. The role requires full access to patient health information and collaboration with a wide network of staff, including social work, Patient Access, Admissions, and administrative leadership.
