The RN Hospital Care Manager I serves as a clinical partner dedicated to delivering comprehensive care management services across both Inpatient and Outpatient departments at Intermountain Health Lutheran Hospital. This position is designed to leverage clinical expertise to guide patients through their healthcare journey, ensuring efficient, effective, and patient-focused outcomes.
In this role, you will collaborate seamlessly with patients, families, caregivers, healthcare providers, payers, and community stakeholders. Your work involves developing and implementing tailored care plans that address both medical and social needs, while utilizing motivational interviewing to support patient self-management. You will act as an advocate for patient rights and a key member of multidisciplinary teams, ensuring timely care transitions and adherence to regulatory standards.
