This position serves as a vital coordinator for home health services, ensuring that patient care is delivered according to acceptable standards of practice and company procedures. You will act as a key liaison, facilitating relationships between physicians, referral sources, patients, caregivers, and employees. A core part of your day involves reviewing and approving patient information submitted by licensed professionals during critical milestones such as start of care, recertification, resumption of care, or evaluation visits. Your work ensures that the high-quality, purpose-driven care Enhabit is known for reaches every patient in their home.
