Located at Lowell General Hospital in Dracut, MA, this position serves as a senior clinical expert within the Diabetes and Endocrine department. The role is dedicated to providing professional nursing care and specialized diabetes self-management education to patients and their families, specifically preparing them for discharge home.
The position exists to support patients with complex illnesses and unique learning needs through health teaching and counseling. By promoting wellness and preventing acute and long-term complications, the role ensures that individuals with Type 1, Type 2, steroid-induced, and cystic fibrosis-related diabetes receive comprehensive, family-centric care.
As a senior-level individual contributor, you will function as an expert clinician and diabetes educator. Your day involves coordinating multi-disciplinary initiatives to manage patient care, facilitating age-specific self-management goals, and providing lifestyle coaching through both one-on-one visits and group sessions in the hospital clinic setting.
You will act as a case manager, overseeing the interpretation of laboratory results, ordering therapeutics and durable medical supplies, and coordinating referrals. A significant portion of your time is spent developing and evaluating individualized plans of care, integrating evidence-based practice into clinical workflows, and managing your own patient assignments by delegating effectively to team members.
Beyond direct patient care, you will play a key role in the professional growth of the department by precepting new staff, guiding their transition into the unit, and participating in performance improvement initiatives and departmental committees. This role requires working independently with limited supervision to resolve complex clinical problems and manage processes using specialized knowledge.

Candidates must possess a valid Massachusetts RN license, three years of RN experience, and current Basic Life Support (BLS) certification. Certification in Diabetes Education (CDE) is required or must be obtained within one year of hire.
