About the Role
Located at Lowell General Hospital within the Diabetes and Endocrine department in Dracut, MA, this position serves as a senior clinical expert dedicated to transforming patient outcomes through specialized diabetes care. The role is designed to bridge the gap between acute hospital treatment and long-term self-management, ensuring patients and their families are fully prepared for life after discharge.
As a Clinical Nurse and Diabetes Educator, you will function as a primary resource for patients with complex illnesses, including Type 1, Type 2, steroid-induced, and cystic fibrosis-related diabetes. Your day-to-day involves orchestrating a family-centric, multi-disciplinary approach to care. You will conduct one-on-one visits and group sessions to provide lifestyle coaching, interpret laboratory results, and manage therapeutic needs. Beyond direct patient care, you will act as a mentor, guiding the professional growth of new staff and precepting team members to foster a culture of excellence and evidence-based practice within the unit.
