This position is dedicated to providing specialized diabetes self-management education to patients and their families at Lowell General Hospital's Diabetes and Endocrine department in Dracut, MA. The role serves as a critical bridge between hospital care and home life, preparing patients for discharge while addressing complex illness and learning needs. By promoting wellness and preventing acute or long-term complications, the Clinical Nurse acts as an expert clinician and educator within a family-centric, multi-disciplinary program.
The majority of the role involves overseeing the design and delivery of care processes for patients with Type 1, Type 2, steroid-induced, and cystic fibrosis-related diabetes. You will function as a senior-level professional contributor, working independently with limited supervision to resolve complex clinical problems. This includes managing patient assignments, delegating tasks based on team skills, and precepting new staff to facilitate their transition into the unit.
Your day will be a blend of direct patient interaction and strategic care coordination. You will conduct one-on-one visits and group sessions to facilitate age-specific self-management goals, offering lifestyle coaching and health teaching. A significant portion of your time is spent collaborating with outside community resources to ensure comprehensive care continuity.
As the expert clinician for the department, you will interpret laboratory results, order therapeutics and durable medical supplies, and manage prescription refills. You will develop, implement, and continuously evaluate individualized plans of care, integrating evidence-based practice and research into daily clinical activities. Additionally, you will participate in performance improvement initiatives and departmental committees to drive clinical outcomes and professional growth within the team.
Candidates must possess a valid Massachusetts RN license, three years of RN experience, and current Basic Life Support (BLS) certification. Certification in Diabetes Education (CDE) is required or must be obtained within one year of hire. The ideal candidate will have a Bachelor of Science in Nursing (BSN) and the ability to effectively communicate with patients, families, and the healthcare team.
