The RN Case Manager is responsible for providing end-of-life care and hospice services to patients in their home or community setting. This role requires compassion, strong clinical judgment, excellent communication skills, and the ability to support patients and families through highly sensitive and meaningful experiences.
You will assume primary responsibility for a patient and family caseload, assessing, planning, implementing, and evaluating care needs during on-call hours in collaboration with the triage team. Your work involves developing patient care plans that address identified problems and goals, ensuring the use of the 4Ms (What Matters, Medications, Mentation, and Mobility), and providing Age-Friendly Care. You will also assist in arranging the delivery of medications and supplies outside of standard business hours and participate in interdisciplinary team meetings to discuss patient needs.
This is a per-visit role requiring the pickup of 3 shifts monthly. Standard hours are Monday through Friday, 8 am to 5 pm.
