The RN Case Manager is responsible for providing end-of-life care and hospice services to patients in their home or community setting. This role goes beyond clinical duties; it requires compassion, strong clinical judgment, and excellent communication skills to support patients and families through highly sensitive and meaningful experiences.
In this position, you will assume primary responsibility for a patient and family caseload, assessing, planning, implementing, and evaluating care needs. You will direct, supervise, and instruct hospice aide staff in the provision of personal patient care. Your work ensures the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provides Age-Friendly Care, developing care plans that address identified problems and mutually agreed-upon goals.
