The RN Case Manager is responsible for providing end-of-life care and hospice services to patients in their home or community setting. This role requires compassion, strong clinical judgment, excellent communication skills, and the ability to support patients and families through highly sensitive and meaningful experiences.
You will assume primary responsibility for a patient and family caseload, assessing, planning, implementing, and evaluating care needs. Your day will involve collaborating with the triage team during on-call hours, attending interdisciplinary meetings, and developing care plans that address the patient's specific problems and goals. You will also ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) to provide Age-Friendly Care.
