The RN Case Manager is responsible for providing end-of-life care and hospice services to patients in their home or community setting. This role requires compassion, strong clinical judgment, excellent communication skills, and the ability to support patients and families through highly sensitive and meaningful experiences.
You will assume primary responsibility for a patient and family caseload, assessing, planning, implementing, and evaluating care needs. This includes directing and supervising hospice aide staff, developing care plans that address identified problems and mutually agreed-upon goals, and ensuring the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) to provide Age-Friendly Care.
