The University of New Mexico Hospital is seeking dedicated Registered Nurses to join our Care Management Department. This role is designed to bridge the gap between acute care and community living, ensuring patients receive seamless, high-quality support throughout their healthcare journey. Whether in the hospital, outpatient clinics, or through utilization management, you will be the advocate who coordinates care, facilitates safe discharges, and reduces hospital readmissions.

In this patient-facing role, you will support individuals from newborns to seniors during their hospital stay. You will manage a caseload of 25–30 patients, attending daily multidisciplinary rounds alongside providers, therapists, pharmacists, and social workers. Your focus will be on proactive discharge planning, ensuring patients are safely transitioned to the next level of care when medically ready. This position is primarily in-person on assigned units.
Here, you will support patients in the clinic setting, focusing on care plan facilitation and transitioning care from acute settings to the home. You will collaborate with primary and specialty care providers to manage referrals for Durable Medical Equipment (DME), home healthcare, and hospice services. The goal is to help patients remain at home and avoid unnecessary readmissions. This role involves navigating multiple software systems and offers a hybrid work model, with the opportunity to work from home one day per week after six months of successful employment.
This role focuses on ensuring the medical necessity and appropriateness of patient treatments. You will conduct chart reviews, correct billing aspects, and work directly with insurance payors to handle appeals and denials. While there is limited direct patient interaction, you will provide critical support to providers and help recuperate payments for patient care. This position is primarily remote, with the option to attend in-person meetings or trainings as needed.
Similar to the main campus Inpatient role, this position supports patient stays through care coordination and discharge planning. You will be assigned to specific units, managing a 1:25–30 patient ratio and participating in daily rounds with the interdisciplinary team. This role is primarily in-person on the SRMC campus.
