The University of New Mexico Hospital is seeking dedicated Registered Nurses to join our Care Management Department. This role is designed to bridge the gap between acute care and community living, ensuring patients receive seamless, coordinated support throughout their healthcare journey. Whether in the hospital, outpatient clinics, or managing utilization, you will serve as a vital advocate for patients from birth to end-of-life, working alongside a multidisciplinary team of providers, therapists, pharmacists, and social workers.

In this role, you will support patients during their hospital stay by coordinating care, managing discharge planning, and proactively facilitating safe transitions to the next level of care. You will maintain a patient ratio of 1:25-30, attending daily multidisciplinary rounds to collaborate directly with the care team. This position is primarily in-person and requires flexibility for weekend and holiday coverage, including a required on-call rotation.
Focused on the clinic setting, this role facilitates care plans and manages the transition of care from acute settings to the home. You will assist with referrals for durable medical equipment, home healthcare, and hospice services to help patients remain at home and reduce hospital readmissions. The work involves close collaboration with primary and specialty care providers, utilizing various software systems to manage referrals. While primarily based in the clinic, eligible employees may work from home one day per week after six months of employment.
This position supports the medical necessity and appropriateness of patient treatments through chart reviews and billing corrections. You will work directly with insurance payors to handle appeals and denials, ensuring accurate reimbursement while providing limited direct patient interaction. This role is primarily remote after completing an onsite orientation phase, with a schedule that typically includes weekdays and occasional weekend or holiday rotations.
Similar to the main hospital inpatient role, this position supports patient stays at the SRMC campus. You will be assigned to specific units, managing care coordination and discharge planning for patients of all ages. This role involves daily collaboration with social work case managers, assistants, and the broader medical team, with a focus on in-person unit-based care.
