The University of New Mexico Hospital is seeking dedicated Registered Nurses to join our Care Management Department. In this role, you will support patients throughout their inpatient stay, ensuring safe transitions to the next level of care when medically ready. You will maintain a patient ratio of 1:25-30, serving all age groups from newborns to seniors. Your day will involve attending daily multidisciplinary rounds and collaborating closely with providers, therapists, pharmacists, and social workers. This position is primarily in-person, with schedules typically running Monday through Friday, 8:00 AM to 4:30 PM. Weekend, PRN, and holiday availability are required, along with a mandatory on-call rotation of approximately 10 days per year.
Focusing on the outpatient clinic setting, this role facilitates care plans and manages the transition of care from acute settings to the home. You will assist with referrals for durable medical equipment, home healthcare, and hospice services, working to help patients remain at home and reduce hospital readmissions. You will collaborate with primary and specialty care providers, navigating multiple software systems including Cerner/Oracle PowerChart, Microsoft Excel, and electronic faxing. While primarily computer and phone-based, this role offers a hybrid model: after six months of employment and successful competency completion, you may work from home one day per week. Schedules are generally Monday through Friday, 8:00 AM to 4:30 PM, with no weekend or on-call requirements.
This role supports the medical necessity and appropriateness of patient treatments through chart reviews and billing corrections. You will provide provider support, manage appeals and denials, and work directly with insurance payors to ensure proper payment for patient care. Interaction with patients is limited. This is primarily a remote work-from-home position following a successful onsite orientation. Schedules are typically Monday through Friday, 7:00 AM to 3:30 PM, with additional weekend and PRN availability and holiday rotations. Staff may be requested to attend in-person meetings or trainings as needed.
Similar to the main hospital inpatient role, this position supports patient stays through care coordination and discharge planning on the SRMC campus. You will be assigned to specific units, maintaining a 1:25-30 patient ratio and attending daily multidisciplinary rounds alongside social work case managers and other clinical staff. The work is primarily in-person on the assigned unit. Schedules are typically Monday through Friday, 8:00 AM to 4:30 PM, with required weekend, PRN, and holiday coverage, as well as an on-call rotation of approximately 10 days per year.
We offer competitive compensation with shift differentials: 17% for weekday nights, 26% for weekend nights, and 15% for weekend days. Additional incentives include a sign-on bonus and relocation assistance. Compensation is based on factors including experience, education, and organizational considerations.
