About the Role
The Registered Nurse (RN) serves as the experienced professional at the heart of our patient care team, initiating and coordinating comprehensive care plans for individuals requiring skilled and palliative support. This role is dedicated to providing compassionate care to patients and their families, with a specific focus on pain management, symptom control, and end-of-life support.
You will work cooperatively with the attending physician, Medical Director, and the interdisciplinary team to ensure the highest quality of care. Your day involves conducting thorough assessments, updating care plans regularly, and providing essential education and supportive care to both patients and caregivers. As a leader within the clinical team, you will supervise the implementation of care plans, coordinate services across all settings, and ensure compliance with state and federal regulations.

Hiring Process
Candidates selected for this position will be expected to possess a valid driver's license and auto insurance to facilitate travel to patient homes. The role reports directly to the Director of Patient Care Services and involves supervising the clinical team.
