As a Case Manager, you will support patients and families through every step of their care journey. Your role is to coordinate services, connect resources, and develop care plans that reflect each patient's unique needs. By partnering with physicians, nurses, and department leaders, you will help ensure safe transitions, clear communication, and consistent quality across the continuum of care.
You will guide patients and families through program orientation, explaining rehabilitation philosophies, insurance benefits, and discharge criteria. You will coordinate education to encourage participation in treatment planning and develop individualized treatment plans that reflect patient strengths and recovery goals. Through psychosocial assessments and seamless collaboration with the Rehab Program Director and interdisciplinary team, you will promote HCA Healthcare's values of compassion, respect, and excellence in every interaction.
