About the Role
The RN Case Manager is responsible for providing end-of-life care and hospice services to patients in their home or community setting. This role requires compassion, strong clinical judgment, excellent communication skills, and the ability to support patients and families through highly sensitive and meaningful experiences.
You will assume primary responsibility for the patient/family caseload, assessing, planning, implementing, and evaluating care needs during on-call hours in collaboration with the triage team. You will direct, supervise, and instruct hospice aide staff in the provision of personal patient care, ensuring the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and providing Age-Friendly Care.

- check_circleDevelop patient care plans addressing identified problems and mutually agreed upon goals
- check_circleReport patient status and needs to clinical leadership, attending physicians, and hospice physicians

Requirements
- verifiedCurrent unrestricted RN licensure in the state of practice
