The Registered Nurse (RN) serves as the experienced professional responsible for initiating and coordinating comprehensive care plans for patients requiring skilled and palliative care. This role is pivotal in bridging the gap between medical directives and daily patient needs, working cooperatively with attending physicians, the Medical Director, and the broader interdisciplinary team.
In this position, you will regularly review and update care strategies, perform prescribed medical treatments including pain management and symptom control, and conduct thorough assessments. A significant portion of your day involves providing education and supportive care to both patients and their caregivers, ensuring a compassionate and informed care environment.
Reporting to the Director of Patient Care Services, this RN leads the Clinical Team. Success in this role requires a demonstrated competency in professional nursing functions, excellent communication skills, and a sympathetic attitude toward caring for hospice patients and their families. You will be expected to maintain flexibility in scheduling, readily accepting new admissions and emergency assignments as requested.
Candidates must possess a valid driver's license and auto insurance to fulfill the requirements of this position. The role involves maintaining knowledge of state and federal regulations and participating in the Quality Assessment and Performance Improvement (QAPI) program.
