About the Role
As a Care Manager, you will serve as a vital member of an interdisciplinary team, assessing, planning, advocating for, and coordinating patient care from admission through discharge. Your primary focus is ensuring a safe transition post-hospitalization while maintaining patient experience, safety, and quality of care.
You will operate within a Care Management model designed to provide effective transition planning and length of stay oversight. This role requires utilizing performance metrics and adopting best practices to uphold the Mercy mission, values, and service standards.

Requirements
