The Inpatient Registered Nurse (RN) Care Manager provides professional, comprehensive, patient-centric care management services for at-risk patients in an acute care environment. This role exists to ensure the use of appropriate healthcare resources throughout the continuum, delivering the right care at the right time in the right setting.
You will partner with the care team to assess, plan, and coordinate care, managing resource utilization and discharge planning while advocating for patients. Your work ensures regulatory compliance and the documentation of interventions in accordance with the Nurse Practice Act and the standards of practice outlined by the American Case Management Association. By remaining current with best practices and industry standards, you will contribute to ongoing professional development and the delivery of high-quality care.
