About the Role
We are seeking a compassionate and organized Resident Care Coordinator to support day-to-day operations in our senior living community. This role ensures high-quality, person-centered care by coordinating resident services, supporting care staff, and collaborating with physicians, families, and interdisciplinary team members. As a key leader within the care team, you will help foster a safe, supportive, and engaging environment for our residents while ensuring compliance with health standards and best practices.

Key Responsibilities
- check_circleLead and support care team operations in partnership with the Director of Resident Care
- check_circleCoordinate and communicate effectively with physicians, staff, and department heads
- check_circleOversee resident health needs, including vitals assessment and care planning
- check_circleConduct medication cart audits and ensure accurate MAR documentation
- check_circleCoordinate move-ins, assessments, and family care conferences
- check_circleSupport hiring, onboarding, and training of care staff and medication aides
- check_circleMonitor resident files, complete audits, and maintain state compliance
- check_circleCreate and maintain individualized care plans
- check_circleFoster a positive and educational work culture through ongoing coaching
- check_circleCollaborate on daily rounds and follow-ups with residents and families
Requirements
- verifiedHigh School Diploma or equivalent
- verifiedPrior experience in assisted living, long-term care, or similar healthcare setting preferred
- verifiedAbility to read, write, and speak English fluently
- verifiedFamiliarity with electronic medical records and MARs a plus
