About the Role
The Healthguide is a key member of Guidehealth's care delivery model, serving as a vital bridge between patients, their primary care physicians, specialty providers, community resources, and the clinical team. This unique position blends non-clinical patient navigation with foundational clinical insight, designed to help patients overcome barriers to care, strengthen relationships with medical practices, coordinate essential resources, and support whole-person health.
You will engage patients remotely and occasionally in person to help them understand their health needs, navigate complex medical and social systems, close gaps in care, and progress toward optimal health outcomes. In this role, you will directly support Guidehealth's mission to empower patients, reduce the cost of care, and promote efficient, compassionate healthcare delivery by leading with empathy. Your work involves building trusting, ongoing relationships with patients, families, caregivers, and providers while addressing health questions, concerns, and care needs through bi-directional communication.
