About the Role

As a Registered Nurse (RN) in Home Health, you will be a pivotal part of our mission to deliver high-quality care in patients' preferred home settings. This role offers significant autonomy and the opportunity to provide 1:1 patient care, ensuring that every interaction aligns with our vision of excellence.

In this position, you will assume primary responsibility for your caseload, assessing, planning, coordinating, implementing, and evaluating treatment plans for patients and their families. Your day-to-day involves conducting initial and follow-up evaluations to identify physical, psychosocial, and environmental needs, then incorporating this data into comprehensive plans of care. You will serve as a key communicator, reporting significant findings and changes in patient conditions to clinical supervisors, physicians, and facility staff to ensure continuous, safe, and effective care.
