As a Registered Nurse in Home Health, you will play a pivotal role in delivering high-quality care to patients in their preferred home setting. This position is designed for those with a commitment to compassion and a desire to be an industry pioneer. You will be responsible for assessing, planning, coordinating, implementing, and evaluating the plan of treatment for your assigned caseload.
Your day will involve making initial and follow-up evaluations to identify the physical, psychosocial, and environmental needs of patients and their families. You will work autonomously to ensure all activities align with the vision of our leadership while maintaining strict patient privacy and confidentiality. By communicating significant findings to clinical supervisors, physicians, and facility staff, you will help revise care plans and ensure the safety and well-being of those you serve.
