About the Role
This Registered Nurse position is designed to support patient care navigation within the clinic setting at Avera Medical Group McGreevy-7th Avenue. The role exists to bridge the gap between clinical care and patient self-management, ensuring effective education and timely healthcare delivery.
In this position, you will manage care for patients within the primary care practice, working closely with clinic leadership and the medical director to tailor the role to the specific needs of the patient panel. A key focus is identifying high-acuity patients and coordinating their care to achieve optimum outcomes in a safe and cost-effective manner. You will serve as a vital resource for the clinical team, helping to develop and monitor care management processes while supporting the development, maintenance, and reporting of quality measures within the medical home model.
Your day-to-day involves collaborating with physicians, providers, and social services to create comprehensive follow-up plans. You will empower patients and families to achieve maximum wellness by assessing barriers to treatment and promoting self-management. Additionally, you will act as a liaison between the clinic and external agencies, communicating care plans to families, hospital staff, and community support networks as needed.

Requirements
- verifiedRegistered Nurse (RN) license
