About the Role

In this position, you will provide in-home direct patient care according to the provider's prescribed plan of care while closely monitoring progress and recovery. You will serve as a dedicated partner to patients, offering 1:1 care that fosters trust and stability.

Your day will involve assessing and documenting patient conditions, reporting changes in vitals, glucose levels, and other critical indicators. You will administer medications, insulin, and IV fluids, while inspecting and caring for wounds, changing dressings, and assisting with personal hygiene. You will also assess patients for further injuries, infections, or bedsores and deliver appropriate treatments. Throughout your shifts, you will collaborate with providers, aides, and allied health professionals to ensure exceptional patient outcomes.
