About the Role
In this position, you will be the cornerstone of patient care, collaborating with an interdisciplinary team of physicians, advanced practice providers, therapists, and case managers to coordinate and optimize treatment. Your day will involve assessing, planning, implementing, and evaluating individualized nursing care plans while educating patients and families on diagnoses, medications, and self-care strategies.

Whether you are managing IV therapy, wound care, and telemetry monitoring or adapting to varying facility procedures, your role is critical in maintaining patient safety and seamless care transitions.
