In this position, you will be the cornerstone of patient care, collaborating with an interdisciplinary team of physicians, advanced practice providers, therapists, and case managers to coordinate and optimize treatment. Your day will involve assessing, planning, implementing, and evaluating patient care while educating patients and families about diagnoses, medications, and self-care strategies.
You will navigate varying facility procedures and patient populations, demonstrating flexibility by floating between units when necessary. Whether managing IV therapy, wound care, or telemetry monitoring, you will maintain accurate documentation in the Electronic Medical Record (EMR) and participate in handoffs and rounds to support seamless care transitions. This role is designed for professionals who thrive in dynamic environments and are dedicated to fostering a collaborative atmosphere, including supporting the orientation of new staff and travelers.
