About the Role
A-Line Staffing is currently seeking an RN Care Manager to join a major healthcare company. This full-time position offers significant career growth potential within a collaborative and supportive team environment characterized by strong leadership and low turnover.

The role is designed for individuals who thrive in a hybrid setup, balancing field visits with remote work. You will be responsible for developing and facilitating complex care management activities, primarily for members with physical health needs, creating personalized care plans that focus on high-quality, cost-effective outcomes.
Your day-to-day will involve coordinating services to help members maintain independence, conducting 5–7 member visits weekly, and ensuring all documentation is completed within 24 hours. You will start your day at 8:00 AM EST reviewing emails, tasks, and schedules to respond to urgent member needs and coordinate care with providers and community resources. Success in this role requires meeting performance metrics regarding visit volume, documentation timeliness, and process turnaround times while collaborating closely with interdisciplinary teams.
Work Environment & Schedule
This position operates on a hybrid model with a required availability of Monday through Friday, 8:00 AM to 5:00 EST. The role involves field visits 2–3 days per week, with remote work utilized for the remaining time. Attendance is mandatory for the first 90 days of employment.
