A-Line Staffing is currently recruiting for a Registered Nurse Care Manager position in Cuyahoga County, partnering with a major healthcare company. This role offers a unique blend of independence and collaboration, designed for nurses who thrive in a field-based environment while making a meaningful impact on member health outcomes. The position serves as a contract assignment with significant potential for permanent conversion based on performance, attendance, and business needs.
As an RN Care Manager, you will lead complex care management activities for members with primarily physical health needs. Your day will involve developing and managing personalized care plans focused on high-quality, cost-effective outcomes, ensuring members can remain as independent as possible. You will conduct 5–7 field visits weekly, coordinating services and responding to urgent member needs alongside providers and community resources.
Working within a supportive team environment characterized by strong leadership and low turnover, you will balance independent field work with close collaboration on interdisciplinary teams. The role requires a standard business schedule (Monday–Friday, 8 AM–5 PM) with a focus on critical thinking and change management to navigate the continuum of care from pre-admission through discharge.
