DaVita Kidney Care is launching a new service line, DaVita Skilled Nursing Dialysis Services (DSD), to bring industry-leading clinical quality directly to patients within skilled nursing facilities (SNFs). Approximately 10% of all dialysis patients reside in SNFs, and transporting them to dialysis centers three times a week places a significant burden on patients while adding avoidable costs to the healthcare system. This new team aims to rapidly grow this service line, leveraging DaVita's extensive resources to develop the necessary infrastructure and improve patient outcomes.
As a Registered Nurse in the DSD program, you will float between locations in Hampton, Newport News, and Norfolk to provide comprehensive, ongoing care for patients in the SNF setting. This role exists to eliminate the need for patient transport by delivering optimal dialysis care on-site. You will take full responsibility for the patient's treatment plan, troubleshooting issues and making sound clinical decisions independently. While you will work autonomously, you will also partner with SNF staff to coordinate care and supervise Dialysis Technicians to ensure efficiency and capacity goals are met.
The environment is designed for those who thrive in a start-up-like atmosphere. You will be expected to multitask, apply critical thinking to assess and diagnose complex patient needs, and find creative ways to "Get Stuff Done." A core part of your day involves conducting impactful patient and family education, ensuring that every interaction contributes to the patient's well-being and understanding of their treatment.
Candidates must possess a current Registered Nurse (RN) license in the state of practice and a minimum of 12 months of RN experience. The application process includes a review of qualifications, including an Associates Degree in Nursing (A.D.N.) or a Bachelor of Science in Nursing (B.S.N.) from an accredited program. Proficiency in basic computer skills, including MS Word and Outlook, is required.
