About the Role

In this position, you will be the cornerstone of patient care, collaborating with an interdisciplinary team of physicians, advanced practice providers, therapists, and case managers to coordinate and optimize treatment plans. Your day will involve assessing, planning, implementing, and evaluating patient care while educating patients and families about diagnoses, medications, and self-care strategies.

You will thrive in a dynamic environment that requires flexibility and professionalism. Whether you are managing IV therapy, wound care, and telemetry monitoring or supporting the orientation of new staff, you will adapt to varying facility procedures and patient populations. This role is designed for those who are eager to foster a collaborative environment and ensure seamless care transitions through active participation in handoffs, huddles, and interdisciplinary rounds.

Hiring Process
