This position serves as a vital health coordinator within our community-based programs, specifically supporting clients with a history of chemical dependence. You will provide comprehensive medical care and services, acting as the primary health liaison for program participants. Your day-to-day involves reviewing medical histories, conducting assessments for new clients, and managing ongoing care plans in collaboration with an interdisciplinary team. You will oversee LPN and CNA staff, coordinate with primary care physicians and specialists, and ensure efficient medication management and tracking within our wellness centers. This role is essential for bridging the gap between clinical needs and community support, ensuring every client receives culturally competent and effective care.
