This role provides essential nutritional services for in-center and home patients, aiming to maximize nutritional status and improve clinical outcomes. As a vital member of the interdisciplinary healthcare team, you will support Fresenius Medical Care North America's (FMCNA) commitment to the Quality Enhancement Program (QEP) and Quality Assessment and Performance Improvement (QAI) activities. Your work actively drives process improvements that help patients achieve their individualized, physician-determined goals.
You will serve as a Registered Dietitian specializing in renal nutrition, assessing patient knowledge regarding diet and kidney disease to provide tailored education. Your day-to-day involves conducting comprehensive nutritional assessments, developing individualized plans of care, and calculating precise diet prescriptions. You will collaborate closely with physicians, nurses, and case managers to manage complex aspects of patient health, including mineral metabolism, anemia, vascular access, and fluid status.
A significant portion of your time will be dedicated to ongoing counseling and outcome-oriented nutrition therapy. You will monitor adherence, review lab results such as albumin levels and body weight, and adjust treatment plans as needed to ensure patients sustain effective nutritional status. Additionally, you will coordinate pre-ESRD education and consult with corporate dietitians on complex nutrition concerns.
