This position coordinates and monitors outpatient nutrition counseling services for clients, serving as a vital link between nutritional science and patient well-being. You will conduct comprehensive nutrition assessments and screenings, develop personalized diet plans, and provide ongoing counseling to support patient health goals.
In this role, you will act as a liaison between patients and their personal physicians, ensuring seamless communication and accurate documentation within medical records. Beyond direct patient care, you will serve as an educational resource for the healthcare team, dietetic interns, families, and the community. This includes presenting nutrition-related classes, seminars, and workshops focused on heart and cancer prevention, senior wellness, and fitness.
You will exercise independent judgment on moderately complex cases while assisting management in developing new clinical services and process improvements. Your work will directly contribute to the mission of providing skilled, effective clinical care that addresses the diverse needs of our patients.
