About the Role

Based at All Saints Hospital in Racine, WI, this full-time Case Management position operates on a day shift schedule. In this role, you will coordinate the overall interdisciplinary plan of care for patients from admission to discharge. You will architect comprehensive care plans that bridge the gap between hospital and home, ensuring every step aligns with the patient's personal values and recovery goals. Your work involves advocating fiercely for patients by identifying and removing systemic barriers to care, ensuring they have the essential resources needed for a safe transition. Additionally, you will orchestrate clinical communication across the multidisciplinary team to ensure the health plan is executed seamlessly and champion documentation integrity by serving as a clinical resource.
