As a Referral Coordinator, you serve as the first point of contact for participants, providing a warm, authentic, and compassionate welcome to our services. Your primary mission is to guide individuals through our triage and assessment process, ensuring their unique needs, preferences, and goals are carefully managed. You will play a pivotal role in helping participants access the right intervention at the right time and in the right way, while encouraging them to utilize our virtual care platform for a self-led journey.
In this position, you will support participants in making informed choices about their care pathways by exploring in-house, sub-contracted, and adjacent service options. Whether engaging via telephone, text, email, or our digital platform, you will facilitate a friction-free experience that respects the dignity and vulnerability of every individual. This role is essential to ensuring that our service remains accessible to all residents, including shift workers and families with school-age children, through flexible working hours.
