About the Role
As a Referral Coordinator, you serve as the first point of contact for our participants, providing a warm, authentic, and compassionate welcome to our services. Your primary mission is to guide individuals through our triage and assessment process, ensuring their unique needs, preferences, and goals are carefully managed. You will play a pivotal role in helping participants access the right intervention at the right time and in the right way, while actively encouraging them to utilize our virtual care platform for a self-led journey.
This position involves supporting participants in making informed choices about their care pathways by exploring in-house, sub-contracted, and adjacent service options. Whether through a professional service delivery or compassionate communication, you will ensure a friction-free experience that supports positive outcomes for families and individuals navigating health and lifestyle changes.
