As a Referral Coordinator, you serve as the first point of contact for our participants, providing a warm, authentic, and compassionate welcome to our services. Your primary mission is to guide individuals through our triage and assessment process, ensuring their unique needs, preferences, and goals are carefully managed. You will play a pivotal role in helping participants access the right intervention at the right time and in the right way, while actively encouraging them to utilize our virtual care platform for a self-led journey.
In this position, you will support participants in making informed choices about their care pathways by exploring in-house, sub-contracted, and adjacent service options. Whether through a professional service delivery or compassionate communication, your focus is on creating friction-free referral pathways that foster a positive experience. You will manage a proactive caseload, engaging with participants via telephone, text, email, and our virtual platform to drive successful onboarding and ensure no one is left without support.
