About the Role
As a Referral Coordinator, you serve as the warm, authentic, and compassionate first point of contact for our participants. Your mission is to guide individuals through our triage and assessment process, ensuring their unique needs, preferences, and goals are carefully managed. You will play a pivotal role in helping participants access the right intervention at the right time, while encouraging them to engage with our virtual care platform for a self-led journey.
In this position, you will support participants in making informed choices about their care pathways, exploring a range of options including in-house, sub-contracted, and adjacent services. Whether through face-to-face interaction or telephone support, your approach will be personable, non-judgmental, and sensitive, fostering a positive experience for every individual you assist.
