As a Referral Coordinator, you serve as the warm, authentic, and compassionate first point of contact for our participants. Your mission is to guide individuals through our triage and assessment process, ensuring their unique needs, preferences, and goals are carefully managed. You will play a pivotal role in helping participants access the right intervention at the right time, in the right way, while encouraging them to utilize our virtual care platform for a self-led journey.
In this position, you will support participants in making informed choices about their care pathways by exploring in-house, sub-contracted, and adjacent service options. Whether engaging via telephone, text, email, or our digital platform, your focus is on delivering a friction-free experience that empowers individuals to navigate their health and wellbeing journey with confidence.
