As a Referral Coordinator, you serve as the first point of contact for participants, providing a warm, authentic, and compassionate welcome to our services. Your primary mission is to guide individuals through our triage and assessment process, ensuring their unique needs, preferences, and goals are carefully managed. You will play a pivotal role in helping participants access the right intervention at the right time and in the right way, while encouraging them to utilize our virtual care platform for a self-led journey.
In this position, you will support participants in making informed choices about their care pathways by exploring in-house, sub-contracted, and adjacent service options. Whether engaging via telephone, text, email, or our digital platform, you will foster a personable, non-judgmental, and sensitive approach to communication. Your work directly impacts the quality of life for families, requiring a genuine desire to support vulnerable individuals through significant change processes.
Your day involves receiving and processing service referrals through multiple channels, including email, text, web forms, and telephone. You will conduct effective triage and assessment, capturing accurate data and identifying any accessibility considerations or safeguarding concerns. A key part of your workflow is managing the referral inbox and onboarding participants onto interventions, ensuring a friction-free experience.
You will also engage in proactive caseload management for unassigned participants, driving successful onboarding through responsive engagement. This includes delivering brief interventions and advice in line with government guidelines, maintaining accurate records, and managing dashboards to track referral numbers and participant journeys. You will work within a framework of Standard Operating Procedures, adhering to strict data protection, information security, and health and safety policies.

We offer a competitive salary of £24,570.00 and require a commitment to flexible working, including evenings and weekends, to ensure our service remains available to all residents, including shift workers and parents.
