As a Referral Coordinator, you serve as the first point of contact for our participants, providing a warm, authentic, and compassionate welcome to our services. Your primary mission is to guide individuals through our triage and assessment process, ensuring their unique needs, preferences, and goals are carefully managed. You will play a pivotal role in helping participants access the right intervention at the right time, encouraging them to utilize our virtual care platform for a self-led journey while supporting those who require alternative pathways.
In this position, you will facilitate informed choices regarding care options, exploring in-house, sub-contracted, and adjacent service possibilities. Your day-to-day involves processing referrals across multiple channels, managing a caseload of unassigned participants, and ensuring accurate data capture while maintaining a sensitive, non-judgmental approach to communicating with the public. You will work within a high-functioning team to deliver brief interventions and advice in line with government guidelines, ensuring a friction-free experience for every participant.
