As a Referral Coordinator, you serve as the warm, authentic, and compassionate first point of contact for our participants. Your mission is to guide individuals through our triage and assessment process, ensuring their unique needs, preferences, and goals are carefully managed. You will play a pivotal role in helping participants access the right intervention at the right time and in the right way, while encouraging them to utilize our virtual care platform for a self-led journey.
In this position, you will support participants in making informed choices about their care pathways, exploring a range of options including in-house services, sub-contracted providers, and adjacent services. Your work ensures that every individual receives a professional, friction-free experience as they navigate their path to better health.
